Kate's Senior Care Minute

# I Wonder If She Knew

This week I had the opportunity to tour an assisted living community that was absolutely beautiful. Honestly, it was one of the nicest communities I've ever visited.

The apartments were warm and inviting, the grounds were immaculate, and the entire community had a feeling that was hard to describe. I actually caught myself thinking, *I could move in here today.* It was that beautiful.

When I arrived, another family was already there. They were sitting together in a glass-enclosed room just off the lobby. The door was slightly open. I wasn't trying to listen, but it was hard not to notice them as I walked by.

An older woman, probably in her late seventies or early eighties, sat quietly with her daughter and son. I don't know their story, and I won't pretend that I do. I don't know what happened that brought them there. Maybe this decision had been discussed for months. Maybe it hadn't. I simply don't know.

As my tour continued, I saw them again.

The woman was sitting quietly at a table while her daughter spoke with a staff member nearby. Then I heard her daughter say, "This is where you're going to live now. There's no talking about it. There's no arguing. This is it."

I looked at the woman.

She didn't say a word.

She didn't cry.

She didn't argue.

She just looked around the room.

I couldn't help but wonder...

**Did she know?**

Did she wake up that morning thinking she was simply spending the day with her children? Or did she know this was the day her life was about to change?

I'll never know.

I wasn't there long enough to know what had brought her to that moment, but it didn't take long to realize this was going to be a difficult transition for her.

The community wasn't what tugged at my heart. Quite the opposite. If someone had handed me a key to one of those apartments, I probably would have unpacked my bags right then and there.

It was her.

All I wanted to do was walk over and give her a hug.

Not because I knew her.

Not because I knew her story.

But because my heart hurt for her.

Her expression said more than words ever could. I could see she was trying to understand what was happening.

Seeing people like her makes my job harder.

Maybe that's a good thing.

Because the day this job stops touching my heart is the day I should stop doing it.

People often ask me what I do. I usually tell them I help seniors and their families navigate one of life's biggest transitions. What I don't always tell them is that sometimes I carry those moments home with me.

I still think about that woman.

I don't know her name.

I don't know if she's settled in.

I don't know if she's made friends.

I don't know if she's found a favorite chair in the dining room or a sunny spot where she enjoys her morning coffee.

I hope she has.

I hope she found kindness.

I hope someone took the time to sit with her, learn her story, and help her feel at home.

I'll probably never see her again.

But I'll always wonder...

**Did she know?**

Community Spotlight

Outstanding Community Outreach - Encompass Rehabilitation Hospital

## Community Spotlight: Encompass Health Rehabilitation Hospitals – Toms River & Tinton Falls

When someone experiences an illness, injury, or surgery, the next step in recovery can be just as important as the care they received in the hospital. That's why I wanted to spotlight Encompass Health Rehabilitation Hospitals in Toms River and Tinton Falls and, in particular, their Direct Admission Program.

Many people don't realize that some patients may qualify to be admitted directly to an inpatient rehabilitation hospital without first being admitted to an acute care hospital, provided they meet the clinical criteria. Through Encompass Health's Direct Admission Program, eligible patients can be evaluated and admitted directly from home, a physician's office, an emergency department, or another healthcare setting. This can help patients begin intensive rehabilitation sooner while avoiding an unnecessary hospital stay when appropriate.

Their admissions team works closely with physicians, hospitals, case managers, and families to determine whether inpatient rehabilitation is the right level of care and to coordinate a smooth transition into the program.

Once admitted, patients receive individualized care from an experienced interdisciplinary team that includes rehabilitation physicians, nurses, physical therapists, occupational therapists, speech-language pathologists, case managers, pharmacists, and dietitians. The goal is simple: help each patient regain as much independence as possible and safely return to the highest level of function.

At Guiding Hands Senior Placement Services, I appreciate having knowledgeable community partners who offer innovative solutions that can improve patient outcomes and make the transition through the healthcare system easier for families. Encompass Health's Direct Admission Program is one of those resources that deserves more attention because it provides another pathway to the specialized rehabilitation many patients need.

If you or someone you know is recovering from a stroke, orthopedic surgery, neurological condition, or another illness or injury requiring intensive rehabilitation, it may be worth asking whether a direct admission to Encompass Health is an option.

Knowing your options can make all the difference in your recovery journey.
